What is a Supplemental Claim?
A Supplemental Claim is a VA decision review option that allows you to submit new and relevant evidence that was not previously considered as part of your claim.
What Can Be Considered New and Relevant Evidence?
New and Relevant Evidence may Include:
- New medical records
- A medical opinion or nexus letter
- Additional service records
- Personal statements
- Buddy statements from family members, friends, or fellow service members
- Other documentation that supports your claim

Authorization to Disclose Information to the VA
If you received treatment from civilian or private healthcare providers for any of the conditions you are claiming, use this form to authorize the VA to request those medical records.
General Release for Medical Provider Information
Use this form to list the civilian or private healthcare providers whose medical records you have authorized the VA to obtain in support of your claim.
Declaration of Status of Dependents
If you are married and/or have dependent children, use this form to provide the VA with information about your dependents. Veterans with a qualifying disability rating of 30% or higher may be eligible for additional monthly compensation for eligible dependents.
Statement in Support of Claim for PTSD
If you are claiming post-traumatic stress disorder (PTSD), use this form to provide information about the traumatic event(s) related to your claim. Complete this form in addition to any personal statement or other supporting evidence you submit.
What Happens Next?
Once your Supplemental Claim and Evidence are Submitted
The VA will review the new and relevant evidence along with the information already associated with your claim. The VA may request additional information, schedule a Compensation & Pension (C&P) examination, or issue a new decision based on its review.
